refactor: handle ignored file operation errors - #545

Replace unchecked Close/Remove/Write-style calls with explicit error handling or intentional ignored-error wrappers. Runtime paths now propagate meaningful errors where it matters and cleanup paths either log failures or explicitly discard errors to satisfy linting while preserving behavior.
